  • 21、A growing number of young people are having treatment for pain caused by taking pictures of themselves for social media. Jokingly named “ selfie (自拍) wrist ”, the condition is actually called CTS — carpal tunnel syndrome (腕管综合征).

    【1】 If this is done repeatedly, the median nerve and blood vessels (血管) which travel through the carpal tunnel into the hand will become compressed (压缩), causing pain and numbness in the wrist and hand.

    【2】 The most important thing is avoiding or reducing the activities that cause it. Meanwhile, wearing splints (夹板) and doing stretching exercises for the hand can work for mild symptoms, though it may take some time to improve the condition. 【3】 For example, a woman who regularly took selfies was found unable to hold things tightly due to severe numbness. As a result, the doctor had to operate on her to relieve pressure within the carpal tunnel.

    In an interview, Dr. Raj Ragoowansi, a consultant plastic surgeon, said he had seen a 30% to 40% increase in people between the ages of 18 and 35 showing symptoms of CTS. He stressed that he could not prove that CTS symptoms were definitely caused by taking too many selfies.

  • 22、Mountains of smoke and fire

    Deep under the Earth’s surface, it’s so hot that even rock melts. Sometimes this molten rock, called “magma”, is pushed up to the surface.1And the opening or vent that lets the lava out is a volcano.

    A volcano may explode violently, throwing out rocks for miles around.2Some volcanoes release clouds of poisonous gas or huge clouds of ash. Volcanoes can even do all these things underwater.

    Most volcanoes have been around for a very long time. Many haven’t erupted for years and have cooled off. Volcanoes that are not going to erupt again are called “dead volcanoes”. Some volcanoes still give off smoke. These “sleeping volcanoes” may “wake up” one day and erupt again. 3 But one day in AD 79 it suddenly woke up. Its eruption threw out hot ash and rocky fragments (碎片) that buried the city of Pompeii. A hot mud-flow buried nearby Herculaneum. Because the remains are so well preserved, the area has been named a World Heritage site.

    But not all volcanoes are destructive. When a volcano throws out vast amounts of lava and debris (岩屑), it piles up into a mountain.4

    Other volcanoes help provide heat and energy. Many Icelandic homes get their hot water from springs heated by volcanic steam.5 Plants grow very well in the rich soil left by volcanoes. And valuable gems, such as diamonds, can sometimes be found in the rocks that are thrown out by volcanoes.

  • 23、   “New and improved. “These words are put in so many marketing campaigns that we tend to accept them as linked. But many new drugs aren’t an improvement over the best existing drug for a given condition, and the fast drug-approval processes in recent years have added to the uncertainty about their advantages.

The authors looked at 216 drugs approved between 2011 and 2017:152 were newly developed, and 64 were existing medicine approved for new uses. Only 25%offered a major advantage over the established treatment, and fully 58%had no confirmed added benefit to reduce symptoms or improve health-related quality of life.

    “This doesn’t mean there’s no added benefit, “lead author Wissler said. “It just means we have no positive proof. Either we have no studies or have studies not good enough. “Wissler and her co-authors work for a German institute which evaluates new treatments and advises on whether the country’s health care system should pay a premium(补贴)for them. Such organizations, known as health technology assessment(HTA)agencies, work a little differently in the US, says Sean Tunisia researcher in Baltimore: “If payers think a new drug isn’t better than an existing drug, these agencies will require that hospitals try the cheaper drug first.”

    Germanys HTA demands trials to prove that a new treatment beats the existing standard. This isn’t always practical. For one thing, such studies can be expensive and time-consuming, with no guarantee of success. Secondly, it can discourage companies from attempting to develop new alternatives. This is already happening. Drug developers are increasingly focused on areas where there are no good treatments to compete with, such as rare diseases.

    This lack of meaningful data to guide patients is a major point of Wissler’s paper. With accelerated approval, there are more products approved, with a greater amount of uncertainty about risks and benefits. But there are other solutions besides drug trials. One idea is to require postmarked studies to track the effectiveness of newly approved drugs—a step too often neglected.
